Case Management

Case Management is required when comprehensive solutions are necessary. Working with the management team, Case Management handles the ongoing coordination and assessment of all medical professionals, community agents, and funding sources to achieve optimal support for the client and their family.

Community Agencies

We provide Registered Nurses, Registered Practical Nurses and Personal Support Workers for Community Agencies such as The March of Dimes, Veterans Affairs.and the Lyons Club Guide Dog Program. We also partner with Health Care Agencies that provide CCAC Services.

Facilities

Hospitals, Long Term Care Facilities and Retirement Homes sometimes face staffing shortages. Our health care staff are available for short or long-term placement to fill staffing needs.

The Legal Profession

Looking for a reliable, competent, health care provider to serve your clients? We provide attendants for Brain Injury and Traumatic Injuries. Our staff have been trained to care for the special needs of this population.

Occupational Therapy

The Occupational Therapist’s assessment allows us to establish a care plan that addresses the areas of required assistance. Our staff provides therapeutic and behavioral support, collecting data to determine the effectiveness of support. Strategies are reviewed as the client progresses towards independence.

Nursing / Home Support

We provide activation, personal care and home making services to assist clients to remain at home, whether that is their house, assisted living, or retirement home. Assistance can be provided for a short term or long term depending on individual needs.

Attendant Care

After a major injury clients may require support at home. Attendants provide assistance with Activities of Daily Living. As part of the health care team, their goal is to establish routines that will lead to client independence.

Acquired Brain Injury

At Alert / Best we recognize the debilitating effects for patients and families dealing with Acquired Brain Injury. ABI is a growing epidemic in our society, due to advances in medical interventions that were not present decades ago. Individuals are surviving accidents that in the past possibly would have taken their life. The effects of brain injury, its severity and treatment vary on a case-to-case basis. Alert / Best is able to cater to the specific individual needs of both the patient and their family support system. We recognize the immediate alteration of day-to-day life and the need for on-going rehabilitation to assist with re-integration into all aspects of daily living. Physical, emotional, and social challenges can present themselves. Catastrophic Injury

Serious injuries, those which cause permanent disability or disfigurement, can require an immediate and an ongoing support system. Alert /Best has highly trained staff to assist with injuries such as, but not limited to:

The diagnosis of a chronic illness comes with the need for understanding, acceptance and implementation of measures to prolong and provide extended quality of life. At Alert / Best, our staff are experienced and educated in supporting individuals and families dealing with chronic illness. Our specialized caregivers can provide assistance within facilities or within our client’s home. Geriatric

Mental Illness
